The name of the golf package.
Preferred date & time of ferry departure from Singapore.
Preferred date & time of ferry departure from Batam, Bintan, Sebana Cove or Tg. Belungkor (Desaru). You can check the ferry schedule from the main menu.
Preferred date & tee times required. (Pls note that if the times requested are not available, the next available slots will be allocated.)
Names as in passports.
Date of birth.
Gender.
Passport nos.
Passport expiry dates.
Nationality.
For those who are chose the overnight packages, please also provide:

Check out date.
No. of rooms required.
